Insurance and Payment 

At the time of your visit, either your co-pay, deductible, or self-pay visit fee is due (whichever is applicable). We will do our best to verify your insurance prior to your visit, but sometimes the information obtained is out of date or inaccurate. Ultimately, it is up to you to know and understand your insurance benefits. If there is a claim rejected for any reason, it will be your responsibility to settle your account balance. If I am not in-network with your insurance, I will provide you with a claim form to submit to your insurance company for reimbursement. Please check with your insurance company regarding their policies/procedures for processing reimbursement claims.

Medication Refills 

It is your responsibility to make an appointment before you run out of medication refills. The routine practice of Sarvis Mental Health is to write a prescription to cover your medication needs until your next appointment. If you keep scheduled appointments or reschedule promptly, there should be no need for additional refills. If an exception occurs, please have your pharmacy fax a refill request to (971) 254-2644 at least 5 working days before you will run out of medication. Refills will be considered for patients with a scheduled follow up appointment and on a case by case basis. Please note, we do not process refill requests after hours, on weekends or holidays. Please remember controlled substances cannot be refilled without an appointment and will only be written during office visits, if appropriate.



Missed Appointments

There is a charge for appointments missed, canceled, or changed with less than a two business days’ notice. Please notify us promptly if you are not able to keep your appointment so we may contact someone on the waiting list.  If you miss three appointments without notification you may be subject to dismissal as a patient. We don’t want to do this! So please reach out by email, phone, or leave a message on our secure voicemail.
 

Confidentiality 

Be assured that your history of mental health issues and medical issues will be maintained in a confidential manner, in accordance with state and federal privacy laws.

Email/Phone Correspondence Outside of Appointments 

Please note that medication regimens will not be changed or adjusted outside of scheduled appointments. If you need clarification regarding your treatment plan, you may email a brief note requesting a phone call. Email/phone calls are not a substitute for an in-office visit. 

 

Paperwork Completion

For requested letters, forms, or other correspondence, there is a fee of $25, unless forms are filled out during a visit.
